What this website collects

When you submit a request form we collect your name, telephone number, email address, and the days and times you chose for a callback. Providers may also give a practice name and an optional note.

We deliberately do not collect health information through this website. There is no field for a medication, a date of birth, a prescription number, or an insurance identifier.

What we do with it

Your submission is sent to our staff by email so that we can call you back. It is not stored in a database on this website, and it is not sold, rented, or shared with advertisers.

Cookies and analytics

This website sets no advertising cookies and runs no third-party analytics or tracking scripts. The map on our contact page is loaded from OpenStreetMap.

Hosting

The website is hosted by Vercel, and form notifications are delivered using Google's mail servers. Standard server logs kept by those providers may include your IP address.
